Chev Chelios survives a fall from the sky, sort of. He's in an unknown location, sedated, while various Chinese are harvesting his organs. His heart i...s gone, in an ice chest; a temporary in its place. Chev escapes, knowing only the name of the guy with the ice chest. He calls Doc Miles, an unlicensed cardiologist, who tells him there's only an hour's life in the artificial heart: keep it charged. Chev needs to find his own heart and get to Doc for a transplant. In Crank: High Voltage (2009), Chev Chelios finds himself in a desperate situation after surviving a fall from the sky. He wakes up in an unknown location and discovers that his heart has been stolen and replaced with an artificial one by a group of organ harvesters. With only an hour of life left in the temporary heart, Chev must find his own heart and get to the unlicensed cardiologist, Doc Miles, for a transplant.

Chev embarks on a race against time to track down the shadowy figures responsible for his predicament. Along the way, he is joined by Eve, a stripper; Rei, a prostitute; and Venus, a friend with Tourette's syndrome, who all assist him in his mission. However, Chev's artificial heart constantly needs to be charged, and this becomes a major challenge for him and his unconventional team.

As Chev's frantic pursuit unfolds, he encounters various obstacles and engages in intense action sequences. The movie keeps viewers on the edge of their seats as Chev battles adversaries and navigates through the seedy underbelly of Los Angeles.
